SUMMER CAMP WAIVER & RELEASE FORM (REV. 05/19/2025)

REQUIRED: This waiver must be completed for all campers.

Welcome to Little Princess Spa!
We are delighted to have your child participate in our magical Summer Camp Program. Please read this waiver carefully. Whether this waiver is signed physically or not, your presence at or use of our facility constitutes implied agreement to the full contents of this document.

Assumption of Risk & Liability Waiver

Participation in our summer camp includes a variety of spa-themed and recreational activities, including hair-dos, mini-mani, mini-pedi, facials, crafts, games, and more. These activities are for entertainment only and are not performed by licensed cosmetologists. We do not use powered tools or harsh chemicals, but minor reactions may still occur.

By enrolling your child:

You accept all risks, known or unknown, including injury, allergic reactions, or accidents.
You release Little Princess Spa, its owners, staff, and affiliates from any liability.
You assume responsibility for informing staff of any allergies or special needs.

We strive for a safe, joyful environment. Our staff is trained and attentive, but parents/guardians are responsible for any child they register.

Medical Release & Authorization

In the event of an emergency, I authorize Little Princess Spa to seek medical attention for my child. I understand that:

Reasonable efforts will be made to contact me or an emergency contact.
I am financially responsible for any medical care needed.

Behavior & Safety Guidelines

Campers are expected to:

Follow all rules and instructions.
Behave safely and respectfully.
Remain fully potty trained (no diapers allowed).

We reserve the right to dismiss campers who are disruptive or unsafe. No refunds will be issued for early dismissal due to misbehavior.

Parents will be notified as soon as possible of any behavioral, safety, or medical incidents involving their child.

Support for Neurodiverse Campers

Little Princess Spa is committed to inclusion.
If your child has ADHD, Autism Spectrum Disorder (ASD), or other developmental needs:

Please inform our team in advance.
We may request an aide for one-on-one support if needed.
We are not a clinical or therapeutic setting.
We reserve the right to dismiss a camper if their needs exceed what our environment can safely support.

Our goal is to create a fun and successful camp experience for all children through open communication and reasonable accommodations.

Health & Hygiene

You certify your child:

Is in good health and able to participate.
Has not had a recent illness or contagious exposure.
Children should arrive clean, dressed appropriately, and with necessary daily items (snacks, water bottle, extra clothes).

Consent to Change Outfit
In the event of a spill, accident, or bathroom mishap:

We will contact the parent or legal guardian immediately. A change of clothes must be brought to the spa so the child can change. Staff will not assist with changing; a parent or guardian must provide clean clothing and guidance as needed.

Camp Environment & Limitations

Staff are female and trained in general supervision.
We are not liable for lost or damaged belongings.
No electronic devices are allowed unless authorized.
We are not liable for injuries or incidents that occur due to failure to follow camp rules or supervision protocols.
Surveillance cameras are for internal use only.

A late fee of $5 per minute will be charged for pickups after camp hours.

COVID-19 & Contagious Illness Release

You affirm that:

Your child or household has had no COVID-19 or flu symptoms in the last 14 days.
You release Little Princess Spa from liability related to any virus exposure.

Media & Photo Consent

I authorize Little Princess Spa to use my child’s photo, video, or likeness in media or promotional content. I waive any compensation or claim related to their usage.

Fraud, Payment & Legal

You are liable for fraudulent transactions or bounced payments.
This waiver is binding indefinitely for all visits.
If any clause is deemed invalid, others remain enforceable

Camp registration fees are non-refundable unless otherwise stated in the registration agreement or due to a verified medical condition with documentation from a licensed physician.

Safety & Supervision	What are your staff-to-child ratios?	At least more or less 3 staff members are assigned to every 20 campers.
	Are staff background-checked and CPR/First Aid certified?	All staff are background-checked. While not all are certified, a CPR/First Aid trained supervisor is on site at all times.
	What emergency procedures are in place?	We follow detailed emergency protocols for evacuation, illness, and injury. Parents are contacted immediately in case of emergency.
	How do you handle injuries, illness, or allergies?	Minor injuries are treated with first aid and documented. For illness or allergies, parents will be contacted immediately.
	Is there secure check-in/check-out?	Yes. Doors are locked during camp hours, and ID is required for pickup. Only approved adults on the Authorized Pickup Form may sign out a camper.
Schedule & Structure	What are the camp hours? Is early drop-off or late pickup available?	Camp runs from 9:00 AM – 4:00 PM. Early drop-off or late pickup is not available.
	What does a typical day look like?	Campers enjoy spa services, arts & crafts, group games, snack/lunch time, and themed group activities.
	Do you offer full-day or half-day options?	We offer full-day camps. Half-day options are not currently available.
	Is there a set weekly theme or rotation of activities?	Yes! Each week features a different magical theme with rotating activities.
Activities & Enrichment	What types of activities are offered?	Campers enjoy spa services, arts & crafts, group games, and themed activities.
	Is it mostly indoors, outdoors, or a mix?	All camp activities are held indoors. We do not offer outdoor play.
	Are there field trips or special events?	No field trips are scheduled, but themed events and special surprises may take place at the spa.
	Is screen time allowed?	Campers are not allowed to use personal devices such as phones or iPads. However, limited screen time may be included at camp on our TV—such as watching age-appropriate movies during wind-down time. The majority of the day is dedicated to imaginative play, creative activities, and social interaction.
	Will my child be learning anything (STEM, art, dance, etc.)?	Campers engage in art, music, movement, and creative expression throughout the week.
Meals & Snacks	Do you provide meals or should we pack lunch?	Please pack a nut-free lunch daily. We do not provide meals.
	Are snacks included?	No. Snacks are not provided. Please pack all food items, including snacks, in your child’s lunchbox.
	Are you nut-free or allergy-friendly?	Yes. We are a nut-free facility. Please do not send any peanut or tree nut products.
	Is there a fridge to store lunchboxes?	No. There is no refrigerator available. Please pack lunches with an ice pack in an insulated bag.
Age & Grouping	What age groups do you accept?	We welcome children ages 4–10.
	How are kids grouped—by age, grade, or mixed?	Campers are generally grouped by age, but some activities may include mixed-age collaboration.
	Are siblings kept together or separated? Does my child need to be potty trained?	We will try to keep siblings together if requested, depending on age and group sizes. Yes. All campers must be fully potty trained—no diapers or pull-ups are permitted.
Cost & Registration	What is the weekly cost?	Pricing varies by location and dates. Please refer to your registration confirmation for full details.
	Are there discounts for siblings or multiple weeks?	Ask your location’s front desk or supervisor for available discounts.
	Is there a registration fee?	A one-time registration fee may apply and will be listed at checkout.
	What is your cancellation or refund policy?	We do not offer refunds under any circumstances.
Social & Emotional Wellbeing	How do you handle bullying or behavior issues?	We enforce a zero-tolerance policy for bullying. Positive reinforcement and redirection are our primary behavior tools. Parents will be contacted for serious or ongoing issues.
	What if my child has separation anxiety?	Parents are not permitted to stay. This is a drop-off and pick-up program only.
	Can I talk to or meet my child’s counselor?	Yes. You may request to speak to your child’s lead counselor or the camp supervisor.
Communication & Updates	Do you send daily or weekly updates/photos?	Group photos and general updates may be shared, but we do not send individual reports. Photos may be used for internal updates and occasional promotional purposes.
	How can I contact the camp during the day?	You will receive the direct phone number for the camp supervisor on the first day.
	Do you offer a parent portal or app?	Not at this time. All communication will be via email, phone, or in person.
