Whether you signed the Agreement Contract and/or the liability waiver or not, by entering the property you assume all risk and liability for yourself and every minor you bring in whether the minor(s) is your legal child or not, if the minor(s) is under your care or came in with you, you are fully responsible and liable for her/he/them. The business, affiliates, owners, employees, third party, independent contractors and customers are NOT RESPONSIBLE for injuries, accidents, slip & falls, choking, allergies, missing child, kidnap and or death. YOU knowingly and freely assume all such risks both known and unknown, even if arising from the negligence of the releasees and/or others. By entering the property, you agree to adhere to this notice and assume all liabilities & responsibilities for yourself, your guest(s), minor children/ ward(s) / all minors you bring in whether he/she/they are your legal child(ren) or not, including all participants and non- participants. Non-participants are any and all persons who are inside the spa but not taking part in spa services and activities.

PLEASE DO NOT PAY THIS INVOICE ONLINE, the BALANCE PLUS TIP WILL BE DUE IN PERSON AT THE END of THE PARTY OR APPOINTMENT, TIPS ARE NOT INCLUDED IN THE PRICE OF YOUR PACKAGE. We recommend 20% gratuity for the staff. A SERVICE CHARGE of 5% will be added and charged in your total bill. The service charge is a standard fee that we add to all events held at our venue. It’s used to ensure that our team can provide you with exceptional experience from start to finish. It covers the costs of providing exceptional service and amenities to ensure your event runs smoothly and successfully. This includes dedicated event coordination and management / Professional staff, and event assistants / Use of our venue, including tables, chairs, linens, and decorations / Cleaning and maintenance of the venue before, during, and after the event. Think of it like a concierge service for your event – we take care of all the details so you can focus on enjoying your special occasion.

DEPOSIT AND FINAL PAYMENTS ARE NON-REFUNDABLE.  IF YOU HAVE PURCHASED A TICKET FOR AN EVENT, YOU HAVE PAID YOUR TICKETS IN FULL WHICH WILL NOT BE REFUNDED.  IF YOU PAY FOR THIS INVOICE ONLINE YOUR PAYMENT WILL BE RETURNED TO YOU AUTOMATICALLY, because you are require to pay your remaining balance IN PERSON. ***FOR DAY SPA ONLY*** Thank you for booking your Princess’ appointment at Little Princess Spa Pembroke Pines A 50% deposit is required to book an appointment and hair salon services. Please make sure that the credit card you use for the deposit and balance due is under the name of the person that booked the appointment, as we will be checking the picture id and credit card on the day of the appointment. The deposit is NON-REFUNDABLE AND NON-TRANSFERABLE. The day and time of the appointment cannot be changed once it has been booked. If you do not show up to your appointment you will lose your deposit. If you arrive more than 15 minutes past your appointment time your appointment gets canceled automatically and your deposit will NOT be refunded or changed to another date. You will need to call, reschedule and make a new deposit. If a child comes in for scheduled appointment and DOES NOT want to receive any services, you are still liable to pay the full amount for that package.  APPOINTMENTS ARE NOT ALLOWED TO BRING IN ANY FOOD, DRINKS, CAKE, CUPCAKES, FAVORS OR DECORATIONS. WE DO NOT DRESS UP FOR THE RUNWAY SHOW FOR DAY SPA APPOINTMENT (unless you purchased a DAY SPA package that includes change of clothes, or you purchased our LPS Princess Set that includes a tutu, LPS Shirt, wings, wands, and a tiara). We kindly ask that there is a max of two adults per child. By paying your deposit you have agreed to this invoice and agreement contract. TIPS ARE NOT INCLUDED IN THIS INVOICE; we recommend a 20 % tip of the total of the appointment package for the employees.

*** FOR EVENTS ONLY *** Thank you for booking your Princess’ event at Little Princess Spa Pembroke Pines

If a child comes in for a scheduled event and DOES NOT want to receive any services, you will not receive a refund on your tickets. We kindly ask that there is a max of one adult per child. Please make sure that the credit card to purchase the tickets is under the name of the person that purchased the ticket, as we will be checking the picture id and credit card on the day of the event. We do not change the day and time, location of the event. Events start on the time it was scheduled for and finishes on time, if you arrive late this will deprive from receiving all the services and you will not receive a refund or a credit for it. If you DO NOT SHOW UP to an event that you have purchased a ticket for, you will not be able to change it to another date, time, location. We hold the right to cancel or change the date of an event. By paying your tickets in full you have agreed to this invoice and agreement contract. TIPS ARE NOT INCLUDED; you will have the opportunity to leave a cash tip at the event for the employees.

*** FOR PARTIES ONLY *** Thank you for booking your Princess’ birthday party at Little Princess Spa Pembroke Pines The spa is a private venue for you to enjoy. Depending on your party package, IF MORE THAN a certain number of kids have been confirmed, an additional hour is required; the additional hour is $399. The additional hour MUST be booked when booking the party, you will NOT be able to book the additional hour after the party has already been booked. Little Princess Spa requires a 50% deposit to book the day and time for you which is NON-REFUNDABLE AND NON-TRANSFERABLE, the remaining BALANCE PLUS TIP is due at the end of the party. Tip options are calculated based on the total cost of your party. We recommend a 20% tip for all employees. Please make sure that the credit card you use for the deposit and balance due is under the name of the person that booked the party, as we will be checking the picture id and credit card on the day of the party. We do not change the day and time of the party unless of an EMERGENCY with a 72-hours written doctor’s notice. Once you booked a package you are not allowed to decrease on the package, however, you may upgrade your package. Little Princess Spa staff will call you on Monday or Tuesday of the week of the birthday party date to confirm the number of kids. The number of kids CONFIRMED WILL BE THE NUMBER OF KIDS YOU WILL BE RESPONSIBLE FOR PAYING FOR at the end of the party, this amount cannot be exceeded (i.e. if more kids show up than the number of confirmed kids that was stated to us, we will NOT be able to provide the service(s) on those kids. The number of kids you confirm will determine the number of employees assigned to work at your party. The number of kids you confirmed over the phone that will be attending your party, is the number of kids you will be charged even if they do not show up. Clients have until the latest Wednesday to give us the final number of children. YOU ARE NOT ALLOWED to call us during the weekend or on the day of the party to give us the head count of guests. If unconfirmed guest shows up for your party (ex: you confirmed 10 and 12 shows up) we hold the right not to provide any spa services to these guest(s) as time will not permit. Or we hold the right to change your party package to accommodate the unconfirmed guest(s). We may try our best to fit them in by providing all or only some spa service if time permits. If any of your guest(s) arrives late for the party, some of the spa service that is included in your package will be reduced/cut off. Meaning, your late guest will not be getting all the spa services included in your party package because time will not permit. Your supervisor will let you know which services your late guest will not be getting and which spa service your guest may be getting. If your party starts late (15 minutes or more) because either you arrived late, or the birthday girl arrived late, or majority of your guest arrived late, or for any unforeseen reason(s) not specified here, we hold the right to cut down on spa service or change your party package. Piñatas are allowed but only the pull-on piñatas, no sticks piñatas are allowed. If piñata time is requested by you (customer), only 1 game will be played at the end of the party plus the piñata game. The guest(s) that will participate in the pink lemonade cheers are the ones arriving to the party on time. Cheers are done in the first 10-15 minutes of party start time. Any guest(s) arriving past this time will not participate in the cheers. Girls/Boys may or may not eat all together depending on if time permits, however they all sit and eat together at cake time. Please be mindful that some party packages have dress up, which means the girls will change their clothes. Some of your guest(s) may choose not to do some or all of the spa services or fashion show included in your party package. We try our best to convince them to participate, nonetheless if they still choose not to participate, we will respect their choice and not provide the services, however if a child comes in for scheduled party and DOES NOT want to receive any services, you are still liable to pay the full amount for that service. While we kindly announce and ask that all parents/ guardians/ caregivers refrain from entering the dressing room while staffs are changing the kids and getting ready for our fashion show, you are not prohibiting from doing so, as the dressing rooms have NO DOORS for this reason. The spa is an open floor plan with very few walls (the only enclosed rooms are bathroom, kitchen, office, and storage) so parents/ guardians/ caregivers can see the child(ren) they are responsible for at all times.

If child decides later that they want the services to be provided, we will not be able to accommodate them. We also ask that customers do not form a line for services to be provided. We do not allow for any services to be re-done. We do not have a designated time for the birthday girl to take pictures by the cake table. If this is something you would like to do, please do so during craft time. We will cut the cake as soon as birthday wishes at the flower wall is over. If you would like to take a picture of the cake prior to cutting, please advise our employees. Our employees are responsible for cutting the cake; if you have any special traditions, please let them know ahead of time.  Parents are encouraged to bring girls’ nails polish-free and clipped. Guest(s) that comes in with gel nail polish on their nails (hands and/or feet) will not get the mini manicure and/or the mini pedicure spa service done. We do not remove gel nail polish. Please be advised that if you choose a package and there is a service in that package that the group of children are not allowed to have, we will substitute it for glitter tattoos. Parties start on the time it was scheduled for and finishes on time. Please be mindful that we have a schedule; we have limited time for taking pictures.  YOU ARE NOT ALLOWED to arrive more than 30 minutes before the party is scheduled to commence and YOU ARE NOT ALLOWED to exceed the party end time. Please be advised that there will be an extra $250 fee if you stay at the facility after your party has ended. You can arrive 30 minutes prior to the party to bring in food, cake, drinks, etc. We have 3 white wooden table with tutu skirt around, decorated in front of the spa for you. NO, NOT ALLOWED: ice cream cakes or ice cream, or mousse cake due to refrigeration logistics YOU ARE NOT ALLOWED TO BRING or HANG WALL PANEL OR BACKDROPS, we provide this service, please ask. You are more than welcome to bring loose balloons and table decorations (NO NOT ALLOWED BALLOON ARCHES or COLUMNS). NO NOT ALLOWED confetti, bubbles, explosive candles, hanging of things on the ceiling, no nails, tape or pin on the wall or anything that can damage the wall. We have hooks already in various strategically place (behind the cake table, behind craft table) for your use if you would like to hang any decoration on the wall (excludes wall panel, and backdrop). NO NOT ALLOWED to open gifts at the spa. NO NOT ALLOWED OUTSIDE VENDORS TO COME IN AND DECORATE THE SPA. Little Princess Spa offer table decor with balloon arches. PLEASE ASK THE BOOKING OFFICE. NO NOT ALLOWED to move any furniture around. NO NOT ALLOWED to use your own princess character company.

YES ALLOWED to bring food for the kids and parents (NO saucy / liquid food), wine and champagne for parents, wine and/or champagne glasses will be provided. If you are bringing any drinks make sure you bring ice, an ice bucket will be provided to you. Parents can provide a birthday cake; however, we. YES ALLOWED to bring party favors, cake or cupcakes. Please be sure to bring party utensils/supplies according to the food you are bringing; if your package does not include it. If you DO NOT bring your own party utensils/ supplies and need to use Little Princess’ Spa supplies, we will add a $45 fee to your invoice. Please make sure you tell the pizza company to have pizza delivered 30 minutes after the party scheduled time. By paying your deposit you have agreed to the invoice/agreement contract /waiver. We kindly ask that there is a max of two adults per child. TIPS ARE NOT INCLUDED IN THE PRICE OF YOUR PACKAGE; we recommend a 20% tip total of the party package for the employees.

Little Princess Spa will not issue any refunds or change the day and time of the party, appointment or event. In the event of Force Majeure ONLY Little Princess Spa shall not be deemed in default or otherwise liable under these rules and policies due to its inability to perform its obligations by reason of any fire, earthquake, flood, substantial snowstorm, epidemic, accident, explosion, casualty, strike, lockout, labor controversy, riot, civil disturbance, act of public enemy, cyber-terrorism, embargo, war, act of God, or any municipal, county, state or national ordinance or law, or any executive, administrative or judicial order (which order is not the result of any act or omission which would constitute a default hereunder), or any failure or delay of any transportation, power, or communications system or any other or similar cause not under SITE’S control (hereinafter all of the foregoing is collectively referred to as ‘FORCE MAJEURE’). Notwithstanding the foregoing, SITE shall be permitted to terminate this Agreement with or without notice to USER in the event that USER is prevented from performing hereunder due to FORCE MAJEURE. WE WILL NOT ISSUE A REFUND; we will issue a credit of the deposit paid by you for you to use towards your next party/appointment/event; this only applies if force of majeure occurs. We are located in South Florida, and due to the extremely hot weather, there are times when air conditioners may break down or stop working. This is beyond our control, so we are unable to offer discounts in the event of an AC issue. Rest assured, your party will still go on for the full duration, and all services will be provided. The only downside is that it may feel a bit warmer in the spa.

***DROP-OFF PARTIES/ DAY SPA/EVENTS***

Little Princess Spa will not be responsible for any missing or abducted child. Little Princess Spa hereby informs you that Little Princess Spa is not liable for any of the following scenarios: a child who has been wrongfully taken by a nonfamily, family, or friend perpetrator using physical force, persuasion, or threat of bodily harm; a runaway child, that voluntarily exits the venue for any reason and cannot be found thereafter. This would include any child 17 years of age or younger, or any individual who is above the age of 18 but is intellectually disabled or cognitively impaired and treated as a minor, OR for any unforeseen reason(s) not specified here. We recommend parents or adults hosting the party/day spa/event to be aware of who is picking up each child attending the party/day spa/event. The parents/ legal guardian/caregiver or adults hosting the party are responsible for their kid- guests and dismissing the kids at the end of the party. We are a private party venue with 1 front door entrance where it can be easily monitored who comes in and out. We recommend parents have the contact of every child guest’s pick-up. A child from a previous party is NOT allowed to stay behind alone to wait for a late pick-up. We request that the parents or responsible adults hosting the party/day spa/event wait with them outside the venue if it is past their party time-frame. Safety cameras do not substitute adult supervision. Little Princess Spa Fairies, Supervisor, vendors and any other staff working the party/day spa/event does not have the responsibility of checking who the child came in with and who she or he is leaving with. It is solely the responsibility of the party/day spa/event HOST or the parents/ legal guardian/caregiver who dropped he/she/ they off.

DJ DANCE PARTY, DJ AS AN ADD ON, PHOTOGRAPHER, ROYAL TABLE DECORATION, TABLE DECORATION ADD ON, MAGICAL PARTY PACKAGE, MAGICAL DAY SPA, HIGHNESS PARTY PACKAGE, CHARACTER ADD ON, FACE PAINTER ADD ON, LIMO ADD ON AND ANY OTHER 3rd PARTY VENDORS ADD ON :

Little Princess Spa holds the right to cancel and/ or remove vendor services due to any unforeseen emergency situation that our vendors may run into even at the last minute. We will then remove the vendor charge to your invoice. Please be advised that the customer WILL NOT receive any additional reimbursement or discount on your invoice total and/ or balance for a vendor that does not show up due to any unforeseen emergency situation.

LIMO RESERVATIONS: A non-refundable deposit of $250 is required to lock in a date and time for limo services. If you cancel the limo services your $250 deposit is non-refundable and it cannot be applied as a credit towards any other services or party.

***GIFT CERTIFICATE ONLY***: Must be used at LPS Pembroke Pines ONLY. Cannot be used toward gift certificates or redeemed for cash. No REFUNDS on the gift card. Valid 1 year from purchase date. MUST CALL, to make an appointment, book a party or purchase tickets. If you use a gift certificate to make a deposit for a party, appointment, event or anything that Little Princess Spa offers; the same agreement above applies.

***FACETIME PRINCESS CHARACTER & VIRTUAL ZOOM PARTY ONLY***: Full amount is required when booking a facetime princess character and a virtual zoom party.  The full amount is NON-REFUNDABLE AND NON-TRANSFERABLE. The day and time of the appointment or party cannot be changed once it has been booked. If you do not login to your appointment or party you will lose your full balance. If you arrive more than 5 minutes past your appointment time your appointment gets cancelled automatically and your full payment will NOT be refunded or changed to another date.

LITTLE PRINCESS SPA SERVICES ARE NOT PERFORMED BY A LICENSED COSMETOLOGIST AND IS FOR ENTERTAINMENT PURPOSES ONLY. We do not use powered equipment, cuticles tools or harsh chemicals during our mini manicure and mini pedicure services.  We will attempt to use products that are considered safe for children. However, we cannot guarantee that there will not be an allergic reaction to products being used for skin, hair, and nails. Unfortunately, skin damage or minor allergic reactions can result from these kinds of activities. Little Princess Spa is not responsible or liable for any allergic reaction due to any services, activities, costumes or consuming any food, candy, and beverage before, during or after the party, event, appointment, classes or camp. Little Princess Spa is not responsible for any allergic reaction caused by our party favors cosmetics which includes nail polish, lip gloss, lipstick, eye shadow and nail sticker (or bath bombs), or to any party favors or anything given from Little Princess Spa to any child. Or any skin or hair conditions resulting from the use of our products. It is the responsibility of the guests or parent, guardian, or the responsible adult to inform Little Princess Spa staff of allergies or any dietary restrictions. Little Princess Spa is not liable or responsible for the products we use to perform our spa services. These products, including but not limited to nail polish, eyeshadow, blush, lipstick, lip gloss, glitter, face glitter, glitter tattoos, glow-in-the-dark makeup, temporary tattoos, face painting, and hair color, may stain the skin, hair, or nails. We recommend using acetone instead of nail polish remover. For removing eyeshadow, blush, lipstick, lip gloss, glitter, face glitter, glitter tattoos, glow-in-the-dark makeup, temporary tattoos, and face painting, we recommend using baby oil or makeup remover.  Little Princess Spa is not responsible or liable for any slip or fall accidents or any accidents in general in the event that you or any minor you bring with you (whether a participator or not) experience any accidents or injuries, including but not limited to: falls from furniture or any area, stepping on sharp objects while barefoot, tripping over cords, furniture, or rugs, falling off the runway, burns from warm or hot water during spa services or in the bathroom, burns from black lights, lamps, lighters, birthday cake candles, or any other source, injuries from objects falling from the ceiling or elsewhere, injuries from broken glass. Incidents of any insect or animal bites. Incidents of choking, whether from food or candy served by us, brought in from outside, or any objects or beads in our arts and crafts activities. If a Piñata is included in your event, whether brought in by a party host or you, we are not responsible or liable for any injuries or medical bills resulting from the Piñata activity or any allergic reactions to the candy inside. Additionally, if the Piñata falls and causes injury, including if the candy falls and hits someone on the head, eyes, or any body parts. Little Princess Spa is not responsible for making sure your child gets picked up and dropped off by a parent, legal guardian, friend or family member. Little Princess Spa will not be responsible for any missing or abducted child. Little Princess Spa hereby informs you that Little Princess Spa is not liable for any of the following scenarios: a child who has been wrongfully taken by a nonfamily, family, or friend perpetrator using physical force, persuasion, or threat of bodily harm; a runaway child, that voluntarily exits the venue for any reason and cannot be found thereafter. Little Princess Spa is not responsible or liable for any head lice infection or transmission, whether you or your child(ren) receive hairdo spa services or participate in any activities at LPS. Even if it is assumed that lice were contracted here, we will not cover any lice treatments, offer refunds for the current event or future booking reservations that a deposit has been made on, or provide compensation for any inconvenience. Lice can be transmitted in various ways and locations, making it impossible to confirm with certainty that lice were contracted at Little Princess Spa. Despite our stringent disinfection procedures and individual combs to prevent such issues, external factors may contribute to these situations, and we will not be held liable. Little Princess Spa is not responsible for any transmitted diseases, viruses, head lice and/or bacteria. Little Princess Spa staff are fully trained and will work to ensure that you and your guests are treated in the most respectable manner and in a safe environment. In consideration of being allowed to enter the spa and/or participate in any parties or programs at Little Princess Spa, the undersigned, on his or her own behalf and on behalf of all minors identified acknowledges, appreciates and agrees that: 1) Services can be hazardous and dangerous. I understand that these activities can result in serious injury to the person or child(ren). I assume any and all risk and damage or injury while on the Little Princess Spa premises. 2) I am aware of the risk, hazards and danger of personal injury, allergic reactions, choking, disability and/or death as a result of participation at Little Princess Spa. 3) I have read a copy of the rules of the spa and understand that it is my responsibility to ask questions about them if necessary. I also understand, it is my responsibility to ensure my child(ren) or any child(ren) I bring in whether legally mine or not understands the rules. It is my responsibility to monitor their activities at all times. I will bring any observation of the rules being broken to the attention of a Little Princess Spa staff member. I certify that I and/or my child(ren) or any child(ren) I bring in whether legally mine or not are in good health and that they have no physical limitations, which would preclude safe participation at Little Princess Spa. 4) I am aware that Little Princess Spa and its employees are not responsible for managing who picks up a child, and/ or who a child leaves the party, event, day spa, class, or camp with. It is my duty or that of the parents / adults hosting the party or day spa, to supervise the children exiting the venue. In any case of child abduction or run-away child Little Princess Spa will not be held liable. In consideration for the admission of my child or any child(ren) I bring in whether legally mine or not, I hereby release, waive and forever discharge and covenant not to sue Little Princess Spa and its owners, agents, employees, officers, directors, trustees, binding heirs, executors, administrators, assignees, and all other persons or entities acting on its behalf, from any and all claims, actions, damages, liability, cost or expense including attorney fees which are related to or arise out of or in any way connected to my child(ren) or any child(ren) I bring in whether legally mine or not participation or use of the facility including those that may arise out of the negligence of other participants or visitors. 5) I acknowledge that participation in activities at Little Princess Spa is strictly voluntary. I hereby certify I am over 18 years of age; I have carefully read the foregoing covenants not to sue and acknowledge that I understand and agree to all of terms and conditions within this document. Prior to signing this agreement, I have had an opportunity to ask any questions and I am aware that by signing this contract/waiver and agreement, I am assuming all risks and potential injury while on the premises. Lastly, I expressly represent that I have the authority, expressed or implied, to bring said child(ren) to Little Princess Spa prior to signing this agreement and I am assuming all risks and potential injury or death while on Little Princess Spa premises.

I, the undersigned, parent/ guardian/ caretaker assume all risks and hazards incidental to participating in the activity and hereby waive, release, absolve, indemnify and agree to protect, defend and hold harmless Little Princess Spa, owners, staff and participants for any claim arising out of any injury to myself/ child to the fullest extent allowed by law. I, the undersigned parent/ guardian caretaker of the participant, am fully aware of the potential dangers and risk inherent in this activity, including physical injury, death or other consequences that may arise or result directly or indirectly from participation in all salon and spa services and activities. If any provision of this clause is held unlawful, void, or for any reason unenforceable by a court of competent jurisdiction, then any such provision shall be deemed severable from this clause and shall not after the validity and enforceability of the remaining provisions.

If you are caught stealing anything from our boutique or spa, you will be prosecuted to the fullest extent of the law. Additionally, any fraudulent use of credit cards or counterfeit bills will result in prosecution.

Please be informed that we have cameras installed throughout all areas of the spa. These cameras record both video and audio, and the footage is used internally for quality control and training purposes. Please note that this footage is strictly for internal use and will not be released upon personal request. By signing and entering the spa you consent and agree to the video and audio recordings.

Minor (Child) Photo Release Form:

I, the parent /legal guardian/ caretaker of the child/children mentioned here grant Little Princess Spa my permission to use photographs taken during this event for any legal use, including but not limited to: publicity, copyright purposes, illustrations, advertising and web content. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use. I hereby release, waive and forever discharge and covenant not to sue Little Princess Spa and its owners, agents, employees, officers, directors, trustees, binding heirs, executors, administrators, assignees, and all other persons or entities acting on its behalf, from any and all claims, actions, damages, liability, cost or expense including attorney fees which are related to or arise out of or in any way connected to my child(ren), or any child(ren) I brought in and I am the caregiver to.

Precautionary COVID-19, Coronavirus or Any Other Contagious Virus Liability Release Form

Symptoms of COVID-19 (Coronavirus), Influenza may include: fever, tiredness, dry cough, aches and pains, nasal congestion, runny nose, sore throat, diarrhea.

I understand the above symptoms and I affirm that I and/ or child, as well as all households’ members, do not currently have, nor have experienced the symptoms

listed above within the last 14 days.

I affirm that I and/or the child, as well as all households’ members, have not been diagnosed with COVID-19 (Coronavirus) within the last 14 days.

I affirm that I and/or child, as well as household members, have not knowingly been exposed to anyone diagnosed with COVID-19 (Coronavirus) within the last 14 days.

I understand that Little Princess Spa and its owners, agents, employees, officers, directors, trustees, binding heirs, executors, administrators, assignees, and all other persons or entities acting on its behalf cannot be held liable for any exposure to the COVID-19 (Coronavirus), influenzas, cold, or any contagious virus either before, during or after coming to Little Princess Spa facilities or any other contagion caused by misinformation on this form or the health history provided by each client.

By signing below, I agree to each above statement and release Little Princess Spa and its owners, agents, employees, officers, directors, trustees, binding heirs, executors, administrators, assignees, and all other persons or entities acting on its behalf, from any and all claims, actions, damages, liability, cost or expense including attorney fees which are related to or arise out of or in any way connected to my child(ren) or any child(ren) I brought in and I am the caregiver to from all liability for the intentional or unintentional exposures or harm due to COVID-19 (Coronavirus), influenza, cold, any contagious virus, or contagion.

CONSENT FOR FASHION SHOW PROPS

At Little Princess Spa, we love making every event magical! As part of our Fashion Show Experience, all participating children (including your child, children in your care, and all guests) will have the opportunity to wear our special costume props—such as wands, wings, capes, and, for boys, superhero capes and masks—unless you specifically request otherwise.

  • If you prefer your child(ren) not to use these props, you must verbally inform the party supervisorbefore the event begins.
  • Props are for use during the fashion show onlyand will be collected afterward. They are not to be taken home.

By signing this waiver, you agree and consent to your child(ren) using these props during today’s event and/or any future events at Little Princess Spa. If you are the host of the event, it is your responsibility to inform your guests of this policy.

About the Fashion Show Experience:
The fashion show is a fun surprise for parents, guardians, and caregivers! During the event, children will get ready behind the scenes for 10-15 minutes before their grand runway debut. While we kindly ask that parents refrain from entering the backstage area during this time, please note that:

  • You can still seethe children at all times.
  • Our spa features an open floor plan(except for enclosed spaces like the bathroom, kitchen, office, and storage). There are no doors to the backstage area, ensuring full visibility.

locations with special offers, advertising, and telemarketing messages.

I, authorize Little Princess Spa dba Where Princesses Dreams Come True LLC to charge my credit card on file for agreed upon purchases. I understand that my information will be saved to file for future transactions on my account or any balance dues, if necessary. I agree that the credit card that I’m using to pay is my credit card, my driver license matches with my ID. I agree that I still have a balance due for this party/ appointment which it will be paid at the end of my party/ appointment. If for any reason I fail to pay, I authorize dba Little Princess Spa Where Princesses Dreams Come True LLC to charge my final balance plus 20% gratuity of the total of the package to the same credit card, gift card on file that I provided Little Princess Spa to pay for the deposit of this party/ appointment. I agree not to dispute any deposit, charges, gratuity to my credit card, gift card for the services that were redeemed or were not redeemed to me. By paying the deposit and/or balance in full I acknowledge and agree to the terms and conditions here within and to the terms and conditions of this contract/waiver. I agree NOT to dispute any charges made to my credit card under this contract agreement. Once services have been provided to me and I have paid any and all outstanding balance, my credit card signature for the balance due serves as a confirmation that I was pleased with all the services provided to me and I agree not to dispute all or any charges made to my credit card including any gratuity I willfully gave.

By signing below, I and/or child, agree to each above rule and will comply with the rules. I will bring any observation of the rules being broken to the attention of a Little Princess Spa staff member. This Invoice/contract/waiver once signed/ dated will apply to any date and time (past, present, future) you enter our facility even if you did not update your current/past contract/waiver to a future/current date or the date you are attending. It will serve as an always and permanent waiver of Liability- Release Form- Indemnification Agreement

YOU knowingly and freely assume all such risks both known and unknown, even if arising from the negligence of the releasees and/or others. By entering the property, you agree to adhere to this notice and assume all liabilities & responsibilities for yourself, your guest(s), minor children/ ward(s) / all minors you bring in whether he/she/they are your legal child(ren) or not, including all participants and non- participants. Non-participants are any and all persons who are inside the spa but not taking part in spa services and activities.

This includes dedicated event coordination and management / Professional staff, and event assistants / Use of our venue, including tables, chairs, linens, and decorations / Cleaning and maintenance of the venue before, during, and after the event. ***FOR DAY SPA ONLY*** Thank you for booking your Princess’ appointment at Little Princess Spa Pembroke Pines A 50% deposit is required to book an appointment and hair salon services. Please make sure that the credit card you use for the deposit and balance due is under the name of the person that booked the appointment, as we will be checking the picture id and credit card on the day of the appointment. The deposit is NON-REFUNDABLE AND NON-TRANSFERABLE. The day and time of the appointment cannot be changed once it has been booked. If you do not show up to your appointment you will lose your deposit. If you arrive more than 15 minutes past your appointment time your appointment gets canceled automatically and your deposit will NOT be refunded or changed to another date. You will need to call, reschedule and make a new deposit. If a child comes in for scheduled appointment and DOES NOT want to receive any services, you are still liable to pay the full amount for that package.  APPOINTMENTS ARE NOT ALLOWED TO BRING IN ANY FOOD, DRINKS, CAKE, CUPCAKES, FAVORS OR DECORATIONS. If a child comes in for a scheduled event and DOES NOT want to receive any services, you will not receive a refund on your tickets. We kindly ask that there is a max of one adult per child. Please make sure that the credit card to purchase the tickets is under the name of the person that purchased the ticket, as we will be checking the picture id and credit card on the day of the event. We do not change the day and time, location of the event. Events start on the time it was scheduled for and finishes on time, if you arrive late this will deprive from receiving all the services and you will not receive a refund or a credit for it. If you DO NOT SHOW UP to an event that you have purchased a ticket for, you will not be able to change it to another date, time, location. We hold the right to cancel or change the date of an event. By paying your tickets in full you have agreed to this invoice and agreement contract. TIPS ARE NOT INCLUDED; you will have the opportunity to leave a cash tip at the event for the employees.

*** FOR PARTIES ONLY *** Thank you for booking your Princess’ birthday party at Little Princess Spa Pembroke Pines The spa is a private venue for you to enjoy. Depending on your party package, IF MORE THAN a certain number of kids have been confirmed, an additional hour is required; the additional hour is $399. The additional hour MUST be booked when booking the party, you will NOT be able to book the additional hour after the party has already been booked. Little Princess Spa requires a 50% deposit to book the day and time for you which is NON-REFUNDABLE AND NON-TRANSFERABLE, the remaining BALANCE PLUS TIP is due at the end of the party. Tip options are calculated based on the total cost of your party. We recommend a 20% tip for all employees. Please make sure that the credit card you use for the deposit and balance due is under the name of the person that booked the party, as we will be checking the picture id and credit card on the day of the party. We do not change the day and time of the party unless of an EMERGENCY with a 72-hours written doctor’s notice. Once you booked a package you are not allowed to decrease on the package, however, you may upgrade your package. Little Princess Spa staff will call you on Monday or Tuesday of the week of the birthday party date to confirm the number of kids. The number of kids CONFIRMED WILL BE THE NUMBER OF KIDS YOU WILL BE RESPONSIBLE FOR PAYING FOR at the end of the party, this amount cannot be exceeded (i.e. if more kids show up than the number of confirmed kids that was stated to us, we will NOT be able to provide the service(s) on those kids. The number of kids you confirm will determine the number of employees assigned to work at your party. The number of kids you confirmed over the phone that will be attending your party, is the number of kids you will be charged even if they do not show up. Clients have until the latest Wednesday to give us the final number of children. YOU ARE NOT ALLOWED to call us during the weekend or on the day of the party to give us the head count of guests. If unconfirmed guest shows up for your party (ex: you confirmed 10 and 12 shows up) we hold the right not to provide any spa services to these guest(s) as time will not permit. Or we hold the right to change your party package to accommodate the unconfirmed guest(s). We may try our best to fit them in by providing all or only some spa service if time permits. If any of your guest(s) arrives late for the party, some of the spa service that is included in your package will be reduced/cut off. Meaning, your late guest will not be getting all the spa services included in your party package because time will not permit. Your supervisor will let you know which services your late guest will not be getting and which spa service your guest may be getting. If your party starts late (15 minutes or more) because either you arrived late, or the birthday girl arrived late, or majority of your guest arrived late, or for any unforeseen reason(s) not specified here, we hold the right to cut down on spa service or change your party package. Piñatas are allowed but only the pull-on piñatas, no sticks piñatas are allowed. If piñata time is requested by you (customer), only 1 game will be played at the end of the party plus the piñata game. The guest(s) that will participate in the pink lemonade cheers are the ones arriving to the party on time. Cheers are done in the first 10-15 minutes of party start time. Any guest(s) arriving past this time will not participate in the cheers. Girls/Boys may or may not eat all together depending on if time permits, however they all sit and eat together at cake time. Please be mindful that some party packages have dress up, which means the girls will change their clothes. Some of your guest(s) may choose not to do some or all of the spa services or fashion show included in your party package. We try our best to convince them to participate, nonetheless if they still choose not to participate, we will respect their choice and not provide the services, however if a child comes in for scheduled party and DOES NOT want to receive any services, you are still liable to pay the full amount for that service. While we kindly announce and ask that all parents/ guardians/ caregivers refrain from entering the dressing room while staffs are changing the kids and getting ready for our fashion show, you are not prohibiting from doing so, as the dressing rooms have NO DOORS for this reason. The spa is an open floor plan with very few walls (the only enclosed rooms are bathroom, kitchen, office, and storage) so parents/ guardians/ caregivers can see the child(ren) they are responsible for at all times.

If child decides later that they want the services to be provided, we will not be able to accommodate them. We also ask that customers do not form a line for services to be provided. We do not allow for any services to be re-done. We do not have a designated time for the birthday girl to take pictures by the cake table. If this is something you would like to do, please do so during craft time. We will cut the cake as soon as birthday wishes at the flower wall is over. If you would like to take a picture of the cake prior to cutting, please advise our employees. Our employees are responsible for cutting the cake; if you have any special traditions, please let them know ahead of time.  Parents are encouraged to bring girls’ nails polish-free and clipped. Guest(s) that comes in with gel nail polish on their nails (hands and/or feet) will not get the mini manicure and/or the mini pedicure spa service done. We do not remove gel nail polish. Please be advised that if you choose a package and there is a service in that package that the group of children are not allowed to have, we will substitute it for glitter tattoos. Parties start on the time it was scheduled for and finishes on time. Please be mindful that we have a schedule; we have limited time for taking pictures.  YOU ARE NOT ALLOWED to arrive more than 30 minutes before the party is scheduled to commence and YOU ARE NOT ALLOWED to exceed the party end time. Please be advised that there will be an extra $250 fee if you stay at the facility after your party has ended. You can arrive 30 minutes prior to the party to bring in food, cake, drinks, etc. We have 3 white wooden table with tutu skirt around, decorated in front of the spa for you. NO, NOT ALLOWED: ice cream cakes or ice cream, due to refrigeration logistics YOU ARE NOT ALLOWED TO BRING or HANG WALL PANEL OR BACKDROPS, we provide this service, please ask. You are more than welcome to bring loose balloons and table decorations (NO NOT ALLOWED BALLOON ARCHES or COLUMNS). NO NOT ALLOWED confetti, bubbles, explosive candles, hanging of things on the ceiling, no nails, tape or pin on the wall or anything that can damage the wall. We have hooks already in various strategically place (behind the cake table, behind craft table) for your use if you would like to hang any decoration on the wall (excludes wall panel, and backdrop). NO NOT ALLOWED to open gifts at the spa. NO NOT ALLOWED OUTSIDE VENDORS TO COME IN AND DECORATE THE SPA. Little Princess Spa offer table decor with balloon arches. PLEASE ASK THE BOOKING OFFICE. NO NOT ALLOWED to move any furniture around. NO NOT ALLOWED to use your own princess character company.

YES ALLOWED to bring food for the kids and parents, wine and champagne for parents, wine and/or champagne glasses will be provided. If you are bringing any drinks make sure you bring ice, an ice bucket will be provided to you. Parents can provide a birthday cake; however, we. YES ALLOWED to bring party favors, cake or cupcakes. Please be sure to bring party utensils/supplies according to the food you are bringing; if your package does not include it. If you DO NOT bring your own party utensils/ supplies and need to use Little Princess’ Spa supplies, we will add a $45 fee to your invoice. Please make sure you tell the pizza company to have pizza delivered 30 minutes after the party scheduled time. By paying your deposit you have agreed to the invoice/agreement contract /waiver. We kindly ask that there is a max of two adults per child. TIPS ARE NOT INCLUDED IN THE PRICE OF YOUR PACKAGE; we recommend a 20% tip total of the party package for the employees.

Little Princess Spa will not issue any refunds or change the day and time of the party, appointment or event. In the event of Force Majeure ONLY Little Princess Spa shall not be deemed in default or otherwise liable under these rules and policies due to its inability to perform its obligations by reason of any fire, earthquake, flood, substantial snowstorm, epidemic, accident, explosion, casualty, strike, lockout, labor controversy, riot, civil disturbance, act of public enemy, cyber-terrorism, embargo, war, act of God, or any municipal, county, state or national ordinance or law, or any executive, administrative or judicial order (which order is not the result of any act or omission which would constitute a default hereunder), or any failure or delay of any transportation, power, or communications system or any other or similar cause not under SITE’S control (hereinafter all of the foregoing is collectively referred to as ‘FORCE MAJEURE’). Notwithstanding the foregoing, SITE shall be permitted to terminate this Agreement with or without notice to USER in the event that USER is prevented from performing hereunder due to FORCE MAJEURE. WE WILL NOT ISSUE A REFUND; we will issue a credit of the deposit paid by you for you to use towards your next party/appointment/event; this only applies if force of majeure occurs.

***DROP-OFF PARTIES/ DAY SPA/EVENTS***

Little Princess Spa will not be responsible for any missing or abducted child. Little Princess Spa hereby informs you that Little Princess Spa is not liable for any of the following scenarios: a child who has been wrongfully taken by a nonfamily, family, or friend perpetrator using physical force, persuasion, or threat of bodily harm; a runaway child, that voluntarily exits the venue for any reason and cannot be found thereafter. This would include any child 17 years of age or younger, or any individual who is above the age of 18 but is intellectually disabled or cognitively impaired and treated as a minor, OR for any unforeseen reason(s) not specified here. We recommend parents or adults hosting the party/day spa/event to be aware of who is picking up each child attending the party/day spa/event. The parents/ legal guardian/caregiver or adults hosting the party are responsible for their kid- guests and dismissing the kids at the end of the party. We are a private party venue with 1 front door entrance where it can be easily monitored who comes in and out. We recommend parents have the contact of every child guest’s pick-up. A child from a previous party is NOT allowed to stay behind alone to wait for a late pick-up. We request that the parents or responsible adults hosting the party/day spa/event wait with them outside the venue if it is past their party time-frame. Safety cameras do not substitute adult supervision. Little Princess Spa Fairies, Supervisor, vendors and any other staff working the party/day spa/event does not have the responsibility of checking who the child came in with and who she or he is leaving with. It is solely the responsibility of the party/day spa/event HOST or the parents/ legal guardian/caregiver who dropped he/she/ they off.

Little Princess Spa holds the right to cancel and/ or remove vendor services due to any unforeseen emergency situation that our vendors may run into even at the last minute. We will then remove the vendor charge to your invoice. Please be advised that the customer WILL NOT receive any additional reimbursement or discount on your invoice total and/ or balance for a vendor that does not show up due to any unforeseen emergency situation.

LIMO RESERVATIONS: A non-refundable deposit of $250 is required to lock in a date and time for limo services. If you cancel the limo services your $250 deposit is non-refundable and it cannot be applied as a credit towards any other services or party.

***FACETIME PRINCESS CHARACTER & VIRTUAL ZOOM PARTY ONLY***: Full amount is required when booking a facetime princess character and a virtual zoom party.  The full amount is NON-REFUNDABLE AND NON-TRANSFERABLE. The day and time of the appointment or party cannot be changed once it has been booked. If you do not login to your appointment or party you will lose your full balance. If you arrive more than 5 minutes past your appointment time your appointment gets cancelled automatically and your full payment will NOT be refunded or changed to another date.

LITTLE PRINCESS SPA SERVICES ARE NOT PERFORMED BY A LICENSED COSMETOLOGIST AND IS FOR ENTERTAINMENT PURPOSES ONLY. We do not use powered equipment, cuticles tools or harsh chemicals during our mini manicure and mini pedicure services.  We will attempt to use products that are considered safe for children. However, we cannot guarantee that there will not be an allergic reaction to products being used for skin, hair, and nails. Unfortunately, skin damage or minor allergic reactions can result from these kinds of activities. Little Princess Spa is not responsible or liable for any allergic reaction due to any services, activities, costumes or consuming any food, candy, and beverage before, during or after the party, event, appointment, classes or camp. Little Princess Spa is not responsible for any allergic reaction caused by our party favors cosmetics which includes nail polish, lip gloss, lipstick, eye shadow and nail sticker (or bath bombs), or to any party favors or anything given from Little Princess Spa to any child. Or any skin or hair conditions resulting from the use of our products. It is the responsibility of the guests or parent, guardian, or the responsible adult to inform Little Princess Spa staff of allergies or any dietary restrictions. Little Princess Spa is not liable or responsible for the products we use to perform our spa services. These products, including but not limited to nail polish, eyeshadow, blush, lipstick, lip gloss, glitter, face glitter, glitter tattoos, glow-in-the-dark makeup, temporary tattoos, face painting, and hair color, may stain the skin, hair, or nails. We recommend using acetone instead of nail polish remover. For removing eyeshadow, blush, lipstick, lip gloss, glitter, face glitter, glitter tattoos, glow-in-the-dark makeup, temporary tattoos, and face painting, we recommend using baby oil or makeup remover.  Little Princess Spa is not responsible or liable for any slip or fall accidents or any accidents in general in the event that you or any minor you bring with you (whether a participator or not) experience any accidents or injuries, including but not limited to: falls from furniture or any area, stepping on sharp objects while barefoot, tripping over cords, furniture, or rugs, falling off the runway, burns from warm or hot water during spa services or in the bathroom, burns from black lights, lamps, lighters, birthday cake candles, or any other source, injuries from objects falling from the ceiling or elsewhere, injuries from broken glass. Incidents of any insect or animal bites. Incidents of choking, whether from food or candy served by us, brought in from outside, or any objects or beads in our arts and crafts activities. If a Piñata is included in your event, whether brought in by a party host or you, we are not responsible or liable for any injuries or medical bills resulting from the Piñata activity or any allergic reactions to the candy inside. Additionally, if the Piñata falls and causes injury, including if the candy falls and hits someone on the head, eyes, or any body parts. Little Princess Spa is not responsible for making sure your child gets picked up and dropped off by a parent, legal guardian, friend or family member. Little Princess Spa will not be responsible for any missing or abducted child. Little Princess Spa hereby informs you that Little Princess Spa is not liable for any of the following scenarios: a child who has been wrongfully taken by a nonfamily, family, or friend perpetrator using physical force, persuasion, or threat of bodily harm; a runaway child, that voluntarily exits the venue for any reason and cannot be found thereafter. This would include any child 17 years of age or younger, or any individual who is above the age of 18 but is intellectually disabled or cognitively impaired and treated as a minor.  A non-authorized member of the child’s family, or someone acting on behalf of a family member, takes or fails to return a child. We will NOT be held responsible for any allegations or assumptions of abuse or molestation of any kind and will not be held liable for such claims, whether they are based on perception or otherwise. Little Princess Spa is not responsible or liable for any head lice infection or transmission, whether you or your child(ren) receive hairdo spa services or participate in any activities at LPS. Even if it is assumed that lice were contracted here, we will not cover any lice treatments, offer refunds for the current event or future booking reservations that a deposit has been made on, or provide compensation for any inconvenience. Lice can be transmitted in various ways and locations, making it impossible to confirm with certainty that lice were contracted at Little Princess Spa. Despite our stringent disinfection procedures and individual combs to prevent such issues, external factors may contribute to these situations, and we will not be held liable. Little Princess Spa is not responsible for any transmitted diseases, viruses, head lice and/or bacteria. Little Princess Spa is not responsible for any transmitted diseases, viruses, head lice and/or bacteria. Little Princess Spa staff are fully trained and will work to ensure that you and your guests are treated in the most respectable manner and in a safe environment. In consideration of being allowed to enter the spa and/or participate in any parties or programs at Little Princess Spa, the undersigned, on his or her own behalf and on behalf of all minors identified acknowledges, appreciates and agrees that: 1) Services can be hazardous and dangerous. I understand that these activities can result in serious injury to the person or child(ren). I assume any and all risk and damage or injury while on the Little Princess Spa premises. 2) I am aware of the risk, hazards and danger of personal injury, allergic reactions, choking, disability and/or death as a result of participation at Little Princess Spa. 3) I have read a copy of the rules of the spa and understand that it is my responsibility to ask questions about them if necessary. I also understand, it is my responsibility to ensure my child(ren) or any child(ren) I bring in whether legally mine or not understands the rules. It is my responsibility to monitor their activities at all times. I will bring any observation of the rules being broken to the attention of a Little Princess Spa staff member. I certify that I and/or my child(ren) or any child(ren) I bring in whether legally mine or not are in good health and that they have no physical limitations, which would preclude safe participation at Little Princess Spa. 4) I am aware that Little Princess Spa and its employees are not responsible for managing who picks up a child, and/ or who a child leaves the party, event, day spa, class, or camp with. It is my duty or that of the parents / adults hosting the party or day spa, to supervise the children exiting the venue. In any case of child abduction or run-away child Little Princess Spa will not be held liable. In consideration for the admission of my child or any child(ren) I bring in whether legally mine or not, I hereby release, waive and forever discharge and covenant not to sue Little Princess Spa and its owners, agents, employees, officers, directors, trustees, binding heirs, executors, administrators, assignees, and all other persons or entities acting on its behalf, from any and all claims, actions, damages, liability, cost or expense including attorney fees which are related to or arise out of or in any way connected to my child(ren) or any child(ren) I bring in whether legally mine or not participation or use of the facility including those that may arise out of the negligence of other participants or visitors. 5) I acknowledge that participation in activities at Little Princess Spa is strictly voluntary. I hereby certify I am over 18 years of age; I have carefully read the foregoing covenants not to sue and acknowledge that I understand and agree to all of terms and conditions within this document. Prior to signing this agreement, I have had an opportunity to ask any questions and I am aware that by signing this contract/waiver and agreement, I am assuming all risks and potential injury while on the premises. Lastly, I expressly represent that I have the authority, expressed or implied, to bring said child(ren) to Little Princess Spa prior to signing this agreement and I am assuming all risks and potential injury or death while on Little Princess Spa premises.

I, the undersigned, parent/ guardian/ caretaker assume all risks and hazards incidental to participating in the activity and hereby waive, release, absolve, indemnify and agree to protect, defend and hold harmless Little Princess Spa, owners, staff and participants for any claim arising out of any injury to myself/ child to the fullest extent allowed by law. I, the undersigned parent/ guardian caretaker of the participant, am fully aware of the potential dangers and risk inherent in this activity, including physical injury, death or other consequences that may arise or result directly or indirectly from participation in all salon and spa services and activities. If any provision of this clause is held unlawful, void, or for any reason unenforceable by a court of competent jurisdiction, then any such provision shall be deemed severable from this clause and shall not after the validity and enforceability of the remaining provisions.

If you are caught stealing anything from our boutique or spa, you will be prosecuted to the fullest extent of the law. Additionally, any fraudulent use of credit cards or counterfeit bills will result in prosecution.

Please be informed that we have cameras installed throughout all areas of the spa. These cameras record both video and audio, and the footage is used internally for quality control and training purposes. Please note that this footage is strictly for internal use and will not be released upon personal request. By signing and entering the spa you consent and agree to the video and audio recordings. 

Minor (Child) Photo Release Form:

I, the parent /legal guardian/ caretaker of the child/children mentioned here grant Little Princess Spa my permission to use photographs taken during this event for any legal use, including but not limited to: publicity, copyright purposes, illustrations, advertising and web content. Furthermore, I understand that no royalty, fee, or other compensation shall become payable to me by reason of such use. I hereby release, waive and forever discharge and covenant not to sue Little Princess Spa and its owners, agents, employees, officers, directors, trustees, binding heirs, executors, administrators, assignees, and all other persons or entities acting on its behalf, from any and all claims, actions, damages, liability, cost or expense including attorney fees which are related to or arise out of or in any way connected to my child(ren), or any child(ren) I brought in and I am the caregiver to.

Precautionary COVID-19, Coronavirus or Any Other Contagious Virus Liability Release Form

Symptoms of COVID-19 (Coronavirus), Influenza may include: fever, tiredness, dry cough, aches and pains, nasal congestion, runny nose, sore throat, diarrhea.

I understand the above symptoms and I affirm that I and/ or child, as well as all households’ members, do not currently have, nor have experienced the symptoms

listed above within the last 14 days.

I affirm that I and/or the child, as well as all households’ members, have not been diagnosed with COVID-19 (Coronavirus) within the last 14 days.

I affirm that I and/or child, as well as household members, have not knowingly been exposed to anyone diagnosed with COVID-19 (Coronavirus) within the last 14 days.

I understand that Little Princess Spa and its owners, agents, employees, officers, directors, trustees, binding heirs, executors, administrators, assignees, and all other persons or entities acting on its behalf cannot be held liable for any exposure to the COVID-19 (Coronavirus), influenzas, cold, or any contagious virus either before, during or after coming to Little Princess Spa facilities or any other contagion caused by misinformation on this form or the health history provided by each client.

By signing below, I agree to each above statement and release Little Princess Spa and its owners, agents, employees, officers, directors, trustees, binding heirs, executors, administrators, assignees, and all other persons or entities acting on its behalf, from any and all claims, actions, damages, liability, cost or expense including attorney fees which are related to or arise out of or in any way connected to my child(ren) or any child(ren) I brought in and I am the caregiver to from all liability for the intentional or unintentional exposures or harm due to COVID-19 (Coronavirus), influenza, cold, any contagious virus, or contagion.

CONSENT TO CHANGE OUTFIT CLAUSE –

All child(ren) participating in any event (your child(ren), the children you are a caregiver to and all your/her guest(s)) will change from their original clothes to Little Princess Spa’s costume(s) (such as: shirts, tutus, dresses, wings provided by us) for the fashion show event, unless you verbally request to decline the changing service in the presence of the supervisor of your party or the party you are attending as a guest. We have a dressing room area designated for change of clothes. All clothes will be removed (except for under garments) by Little Princess Spa’s staff(s). We are not responsible for child(ren) that show up without an under garment (underwear/bra). After the fashion show we will dress them back in their original clothes. By signing this contract/waiver, you agree and consent that changes of clothes will take place during the event you are attending here today and/or future events. If you are the HOST of this party/ event, you are responsible for notifying your guests that their child(ren) will change clothes during your event.

The fashion show is meant to be a surprise for the parents/ guardians/ caregivers.  The children change into beautiful fairy outfits to surprise their parents/ guardians/ caregivers as they walk down our runway. This changing process takes anywhere from 10-15 minutes out of the entire 2 or 3 hours of the party, during which you can still see the child(ren) you are responsible for. While we kindly announce and ask that all parents/ guardians/ caregivers refrain from entering the dressing room, you are not prohibiting from doing so, as the dressing rooms have NO DOORS for this reason. The spa is an open floor plan with very few walls (the only enclosed rooms are bathroom, kitchen, office, and storage) so parents/ guardians/ caregivers can see the child(ren) they are responsible for at all times.

I consent to receive calls, emails, and texts from any and all dba Little Princess Spa

locations with special offers, advertising, and telemarketing messages.

I, authorize Little Princess Spa dba Where Princesses Dreams Come True LLC to charge my credit card on file for agreed upon purchases. I understand that my information will be saved to file for future transactions on my account or any balance dues, if necessary. I agree that the credit card that I’m using to pay is my credit card, my driver license matches with my ID. I agree that I still have a balance due for this party/ appointment which it will be paid at the end of my party/ appointment. If for any reason I fail to pay, I authorize dba Little Princess Spa Where Princesses Dreams Come True LLC to charge my final balance plus 20% gratuity of the total of the package to the same credit card, gift card on file that I provided Little Princess Spa to pay for the deposit of this party/ appointment. I agree not to dispute any deposit, charges, gratuity to my credit card, gift card for the services that were redeemed or were not redeemed to me. By paying the deposit and/or balance in full I acknowledge and agree to the terms and conditions here within and to the terms and conditions of this contract/waiver. I agree NOT to dispute any charges made to my credit card under this contract agreement. Once services have been provided to me and I have paid any and all outstanding balance, my credit card signature for the balance due serves as a confirmation that I was pleased with all the services provided to me and I agree not to dispute all or any charges made to my credit card including any gratuity I willfully gave. 

By signing below, I and/or child, agree to each above rule and will comply with the rules. I will bring any observation of the rules being broken to the attention of a Little Princess Spa staff member. This Invoice/contract/waiver once signed/ dated will apply to any date and time (past, present, future) you enter our facility even if you did not update your current/past contract/waiver to a future/current date or the date you are attending. It will serve as an always and permanent waiver of Liability- Release Form- Indemnification Agreement

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
